3、投標人資格要求
投標人應具備的資格或業績:投標人資格和業績要求：
Qualification and Achievement requirements for the bidder.
投標人須提供以下文件
The following documents must be provided.
（1）投標人須按招標文件第一冊第四章格式IV-9.資格證明文件格式的要求填寫和提交有關資料；
All Bidders are requested to fill in according to the requirements of Format IV-9 in Section IV and provide relevant information；
（2）投標產品原產地在關境內的投標人必須是制造商（不接受代理商投標）。
Bidder must be the Manufacturer if the goods are originated from PRC
投標產品原產地在關境外的投標人必須是制造商或制造商直接授權的代理商；
Bidder must be the Manufacturer itself or directly authorized Agent if the goods are originated from outside of PRC；
（3）如為代理商投標：
If as an Agent:
須提供制造商對本次投標出具的唯一正式授權書；
1)The bidder should provide a letter with original sole official authorized by Manufacturer for the bid；
2)投標人在當地依法登記注冊的時間距開標之日須滿三年，并提供營業執照等注冊登記的有效證件復印件；
The Agent shall get registered in accordance with the local law for at least three years from the bid opening date and copies of valid registration documents such as business license shall be provided;
3)開標之日前三年內，投標人須具有同類產品至少兩個不同年份的銷售業績，并提供相應證明其銷售業績的合同或訂單復印件，并由投標人代表簽字或蓋章；
The bidder shall have sales performance for similar products in two different years within three years from the bid opening date. Copies of the contract / purchase order evidencing its sales performance should be provided and signed or sealed by the Bidder;
（4）投標人須提供開戶銀行在開標之日前三個月內開具的資信證明原件或該原件的復印件（由投標人代表簽字或蓋章）。
The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)；
（5）第八章中要求的其他合格的資格證書。
Other Qualification and Achievement requirements stipulated in Chapter VIII.
（6）所投產品的業績
1）業績期限：招標公告發布日期前3個周期年內（以合同簽訂日期為準）。
2）業績范圍：在中國境內應用于雙酚A裝置的成功應用業績（依據業績證明材料確認）。
3）業績數量：至少1份(不包括試用業績）。同一用戶企業內的業績視為1份，不累加。
4）業績證明文件：投標人提供的業績證明文件應包括供貨合同復印件、與供貨合同對應的發票復印件、與供貨合同對應的用戶企業出具的業績證明材料。發票中產品數量應不少于供貨合同中產品數量的30%，發票中的信息應滿足查驗需要。
業績證明材料中應有產品名稱及規格型號，應有使用裝置名稱及生產能力、使用時間、使用數量，應有體現產品性能作用、使用效果等方面的內容，應加蓋用戶企業主管部門公章，應有用戶企業主管部門負責人簽字及聯系方式且可核實。是否接受聯合體投標:不接受
未領購招標文件是否可以參加投標:不可以
